In order to maintain a good patient-physician relationship and avoid misunderstandings we have outlined some of our policies below. Please review each policy attachment in detail to gain a full understanding of our policies and expectations surrounding these topics. Appointments

All appointments are scheduled during normal business hours except New Patient Appointments. New Patient Appointments are scheduled on Tuesdays and Thursdays only. 

Please remember to bring your insurance card, photo ID, copay and any balance payment to your appointment. 

 

Communication

To ensure effective and secure communications between our patients and staff, we have established a Communication Policy. Please note that email, text messaging, and social media platforms or apps are not secure methods of communications. If you communicate to us through these methods, it is our office policy not to respond through these medias.

Billing

In order to provide the best health services, it is our policy to receive payments when services have been rendered. If you have a copay, it is due at the time of service. If you are scheduled for an appointment any prior outstanding balance must be paid at the time of service unless other arrangements were made with our Billing Department.

Prescription Refills 

For better care of our patients, we must monitor the benefits, side effects, interaction of medications, and management of medical conditions of our patients. Clinical evaluations help your physician determine whether continuation, modification, discontinuation, or change in your medications may occur.

Referrals

All referral requests require an Office Visit with Dr. Desai. Most insurances do not allow referrals to be processed retrospectively, so it is the responsibiltiy of the patient to obtain a referral prior to seeking any specialy care. We do not guarantee same-day referrals and are not responsible for any rescheduled specialty appointments or financial liabilities that may incur.

Document Processing

Any document that needs to be completed  by Dr. Desai is subject to a $10 Document Processing Fee. A signed policy and the $10 fee are due at the time of document submission. Typical processing time takes 7-10 business days. All documents must be submitted in their entirety (no missing pages) and patients must complete their portion of the documents before our office will begin processing them.

Medical Records

If you are a patient looking to obtain a copy of your medical records, please complete and return to us a Medical Records Request.

If you are a patient and have a third-party respresentative that needs a copy of your medical records, please have the third-party send us a written request for your records.

If you are a patient and need medical records from another physician sent to our office, please complete a Medical Records Release and send it to the office who holds the records you need transferred.

If you are a patient and need your medical records from our office sent to another physicians' office, please have the requesting physician's office send us a written request for your records.

Under Maryland Health-General Article Section 4-304(c)(3), it is within the physicians' right to charge a Medical Records search fee, cost per page, and dispatch fee for all Medical Records Requests from patients or their 'personal respresentatives'

 
 

MVA Visits

If you have been involved in an auto-accident and plan to file your medical claims through an Auto Insurance, please download and bring a completed MVA Accident Form to your appointment. Our office must have all claim, carrier, and adjustor information at the time of service in order to file your visit to the auto insurance.  

If your accident claim is still pending or you do not have this information by the time of your appointment, your claim will be filed to the health insurance on-file.

 

Worker's Comp Visits

If you have have been involved in a work-related injury and plan to file your medical claims through your Worker's Comp Insurance, please download and bring a completed Worker's Comp Form to your appointment. . Our office must have all claim and carrier information at the time of the service in order to file your visit to the Worker's Comp Insurance.

If your worker's comp claim is still pending or you do not have this information by the time of your appointment, your claim will be filed to the health insurance on-file. 

 

School Physicals

School-related physical exams should be scheduled at minimum, 2-3 weeks prior to their deadlines. If you are a new patient, you must be seen for a New Patient Appointment before we can conduct a school physcial. Please bring hard-copies of your entire medical records and entire vaccination records to avoid any delays in processing your school physical documents.

DOT Physicals

Dr. Desai does not perform DOT Physicals. Please visit the National Registry of Certified Medical Examiners to find a certified physician in your area.
